Default OBD Faults

Greetings,
I have a 2002 Pathfinder with 165k miles. Went to get her "smogged" today and 4 Fault codes show up. P1167, P0158, P0021 and P0011. After research it looks like changing the Bank 2 Sensor 2 O2 sensor, however I don't know if that is the correct fix for the 1167 and 0158 codes (advice here). I am a bit stumped on the P0021 and 0011 codes...
Thoughts?

P1167 NISSAN - Heated Oxygen Sensor 2 (Rear) Maximum Voltage Monitoring (Bank 2)

It looks like you definitely have a problem with the rear O2 sensor. Both the P1167 and P0158 code are related to that sensor.

For the P0011 and P0021, I suggest starting by changing the engine oil.
