Default Overheating issues...

It's a 2002 Nissan Altima, been overheating for awhile. It can barely make it 3/4 mile without the temperature hitting 260 or whatever the number is in red. You can see smoke appear from underneath the hood from the radiator water blowing somewhere out to the engine. Anybody got any ideas?

-Radiator Cap?
-Radiator Hose?
-Radiator fluid?
(Maybe it needs to be drained and refilled with proper fluid, it might have just too much water in it)
-Thermostat?

Overheating can be caused by defects in the cooling system and elsewhere such as: low coolant level, plugged dirty or blocked radiator, defective fan clutch or electric fan, incorrect ignition timing, low engine oil level, broken fan drive belt, defective radiator cap, dragging brakes, frozen coolant, defective thermostat, defective water pump, clogged cooling passages in the engine block or cylinder head.
