Default 2003 Pathfinder LE front Axle nut loose

Noticed a grinding noise from right front wheel when braking. Inspected brake and found nothing. Pulled front wheel hub apart and found what looked like two pointed pins, 1/4 inch long rolling around in hub. They are actually the screws that hold the locking plate to the axle nut to lock it in place. The front right axle nut had backed off about a full turn leaving alot of slop in the wheel. The wheel bearing was shot but did not damage the axel. Replaced bearings and screws (with loctite) and the noise is gone. Very scary that the screws had fallen out and the nut was backing off. Pathfinder has 61000 miles on it. Anyone else seen this?

I have seen a few, but it's not very common. Usually happens after the bearings has been adjusted and repack.
