Would appreciate some advice on a starter issue, My father has a 1994 Altima and the starter has been replaced twice in the last week, my question is there anything they are missing a spacer or not in aligment, he has stated it will makes a noise when starting but only 1 out of 4 or 5 starts.

Remove that stater and inspect the "Flex Plate" teeth. You have to turn the crank shat by hand to inspect all the teeth. If the teeth are worn out you may have to replaced the flex place to fix the problem.
